What Is Ebola and Why Is Stopping the Latest Outbreak So Difficult?

Ebola is back in the headlines, and for all the wrong reasons. An outbreak in the Democratic Republic of Congo is spreading rapidly, and health officials are warning that containing it will be extraordinarily difficult. But why?

First, a quick reminder of what Ebola is. The virus causes a severe haemorrhagic fever, with symptoms including fever, vomiting, diarrhoea, and internal and external bleeding. It spreads through direct contact with bodily fluids of infected people or contaminated materials. The fatality rate can be as high as 90%, though early treatment significantly improves survival chances.

And it is not like we have not been here before. The 2014-2016 West Africa outbreak killed more than 11,000 people. The 2018-2020 outbreak in eastern DRC killed nearly 2,300. Each time, the world mobilised, contained the virus, and promised to do better next time. And each time, the next outbreak came.

So what makes this outbreak different? Two things: the virus strain and the location.

This outbreak involves a rare species of the Ebola virus for which existing vaccines may be less effective. And it is unfolding in a region already devastated by armed conflict. Fighting between government forces and rebel groups has made it nearly impossible for health workers to reach affected communities. Treatment centres have been attacked. Contact tracing has been disrupted. The combination of a lethal virus and ongoing war is a nightmare scenario.

For Africa, the Ebola story is a painful reminder of the fragility of health systems on the continent and the urgent need for stronger investment in public health infrastructure. Because the next outbreak is not a question of if β€” but when.
